This practice uses text messages to facilitate the care of patients

Purpose: The purpose of receiving text messages is to facilitate communication between patients and the practice to better serve patients.  The communication includes, but is not limited to appointment reminders, test results, prescription updates, and general health information.

SMS Consent : By texting us, you agree to receive text messages at the provided number from Koka Cardiology. 

Consent and Opt-In: By providing your phone number and opting in to receive text messages, you consent to the collection and use of your personal information as described in this policy. We ensure that your consent is obtained explicitly and that you are informed about the types of messages you will receive. You can revoke your consent to receive text messages by calling Koka Cardiology at 215-543-7002 or emailing myphillyheartdoc@gmail.com . 

Opting out of Text Messages: You have the right to OPT-OUT receiving messages at any time. To OPT-OUT, reply "STOP" to any text message you receive from us. Reply HELP for assistance.

Message and Data rates: Standard text messaging and data rates may apply when receiving messages from Koka Cardiology, depending on your mobile service plan.

Privacy: Koka Cardiology will maintain the confidentiality of the patient's personal health information in accordance with applicable laws and regulations.  No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.  All of the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Contact Information: Patients agree to provide Koka Cardiology with their current mobile phone numbers and must notify them of any changes to contact information.

 Termination: Patients may terminate this consent at any time and for any reason, including but not limited to changes in the law or changes in services provided by Koka Cardiology.

Governing Law: This consent is governed by the laws of the state in which Koka Cardiology is located.
